Call us and we will book you at whichever office suits you. Our staff can answer insurance and scheduling questions while you are on the line, which is usually faster than any form.

Please do not include medical details — symptoms, test results, diagnoses, or medication lists — in any online message or email. Email is not a secure way to send health information. Our staff will collect your medical history safely by phone or at your visit.
Online appointment requests — what they will look like
We are not accepting online requests yet. A request form only goes live once a vendor with a signed Business Associate Agreement is in place, because anything you type into it counts as health information. Until then, calling is both faster and safer.
